The list price, explained

Novo Nordisk sets the wholesale acquisition cost (WAC) for Wegovy at approximately $1,349 per monthly package. WAC is the price pharmacies pay Novo Nordisk, before any rebates or discounts. The retail price you see at the pharmacy counter is usually WAC plus a modest pharmacy markup. It is the same across all five dose strengths (0.25, 0.5, 1.0, 1.7, and 2.4 mg), which is unusual — most medications are priced by strength.

In 2025, Novo Nordisk launched a major pricing change: a direct-to-consumer self-pay program through NovoCare Pharmacy that bypasses the traditional insurance and pharmacy-benefit-manager (PBM) system entirely. The self-pay price is $499 per month for any dose. You pay Novo Nordisk directly and the medication ships from a mail-order pharmacy.

This was a direct response to competition from Eli Lilly\'s Zepbound (tirzepatide), which had launched its own LillyDirect self-pay program in 2024, and to political pressure on drug prices. For uninsured patients or people whose insurance excludes weight loss medications, it was a meaningful reduction — roughly 63% off retail.

For patients who use insurance, your out-of-pocket cost depends mainly on your plan — pharmacy-to-pharmacy differences are usually small. For cash-pay patients who want to use a local pharmacy instead of NovoCare, here are typical retail cash prices (not including coupons) as of early 2026:

Wegovy cost with insurance

Coverage varies enormously. Many commercial insurance plans now cover Wegovy, but they typically place it on Tier 3 (non-preferred brand) and require prior authorization. Common prior authorization criteria include:

  • BMI ≥ 30, or BMI ≥ 27 with at least one weight-related comorbidity
  • Documented weight loss attempts (usually 6 months of diet and exercise)
  • Prescribed by a physician (not always a specialist)
  • No contraindications (thyroid cancer history, MEN 2, pregnancy)

If your plan covers Wegovy, your monthly copay is usually between $25 and $100 before any additional savings. Apply the NovoCare Savings Card and many patients pay $0 to $25 per month for up to 12 months.

Medicare and Medicaid

Medicare

Medicare Part D is prohibited by statute (the Medicare Modernization Act of 2003) from covering drugs prescribed solely for weight loss. For most of Wegovy\'s existence, this meant Medicare beneficiaries could not get it covered.

That changed in March 2024 when the FDA approved Wegovy for a new indication — reducing the risk of major adverse cardiovascular events (MACE) in adults with obesity and established cardiovascular disease. Following the approval, CMS clarified that Part D plans are permitted (though not required) to cover Wegovy when prescribed for the cardiovascular indication. Coverage for weight loss alone remains excluded.

Whether your specific Part D plan covers it depends on the plan. Check the plan\'s formulary for "semaglutide (Wegovy)" and look for prior authorization requirements related to the cardiovascular indication.

Medicaid

Medicaid coverage for Wegovy varies dramatically by state. As of early 2026, approximately 13 states cover GLP-1 medications for obesity treatment through Medicaid, including Massachusetts, California (partial), Pennsylvania, and Delaware. The majority of states still exclude obesity medications from Medicaid coverage. Check your state\'s Medicaid preferred drug list or call your state Medicaid agency.

Tricare

Tricare generally does not cover Wegovy for weight loss. It covers semaglutide for diabetes (Ozempic) when medically indicated. Discuss alternatives with your military health provider.

  3. Request prior authorization appeal if your insurance denies the first time. Most denials are reversed after an appeal with complete documentation.
  4. Ask about step therapy — some plans require you to try Saxenda or another medication first. If you have, document it.
  5. Telehealth bundled pricing — some online providers offer flat monthly rates that include the prescription, consult, and medication.
  6. Check HSA/FSA eligibility — Wegovy is an eligible expense with a Letter of Medical Necessity from your provider, reducing the effective cost by your marginal tax rate.
  7. Do not use "compounded semaglutide" from non-licensed sources. The FDA has warned repeatedly about potency, purity, and sterility issues.
About compounded semaglutide

During the 2022-2024 FDA shortage period, some 503A and 503B compounding pharmacies legally produced compounded semaglutide at lower prices. In early 2025, the FDA declared the shortage resolved, and most legal compounding ended. Products still marketed as "compounded semaglutide" today are often sourced from unregulated facilities and carry significant risks. They are not FDA-approved and not bioequivalent to Wegovy.
